Prior to your first Chiropractic visit/Consultation, please download the following Intake Form, complete it and bring with you to your first visit. If you have any medical or x- ray reports, please bring with you to the first visit.

If you do not have the opportunity to complete these forms prior to your first visit, please arrive at the Clinic 15 minutes before your scheduled appointment time. This will allow you time to complete the forms so we can then do a complete and thorough history and examination.
